SyncHealth has been providing strategic leadership since 2013 to help develop primary care systems in the United States that improve the health of populations.

SyncHealth has turned its focus to individuals and organizations in Greece, and potentially other countries, that seek to build strong primary care. Small or larger projects that include medical, broader health, patient and community partners, would be considered. Collaboration with local or national governments would increase the chances of success and sustainability.

Pano Yeracaris, MD MPH is the founder and Chief Strategist for SyncHealth, LLC. He has worked extensively in U.S.  primary care and health system transformation since 1984. Dr Yeracaris graduated with honors from Harvard College in 1977, finished medical school at the State University of NY at Buffalo in 1981, and finished residency in Family Medicine at UNC Chapel Hill in 1984. He practiced and taught from 1984-1997 in Buffalo, NY, and received his Masters in Public Health from the Harvard School of Public Health in 1998. He has lived and worked in the Boston and Rhode Island areas since 1997.

Dr. Yeracaris has over 37 years of medical practice and leadership experience and is the chief clinical strategist for the Care Transformation Collaborative-Rhode Island (formerly CSI-RI), the statewide private/public multi-payer patient centered medical home initiative focused on primary care and health system transformation. (CTC RIMJ article) Additional past engagements include Medical Director for the CPC Plus National Learning Team, and Subject Matter Expert for the Connecticut SIM Primary Care Modernization project.

EXPERIENCE

Prior to this current work, Pano served for nearly nine years as Vice President and Chief Medical Officer at Network Health, a Massachusetts based Managed Medicaid Organization. There he oversaw the entire Clinical Affairs Division and was the architect for its nationally recognized clinical programs including integrated care management and field based social care management and clinical community outreach teams. He also played a substantive role in supporting the MA Patient Centered Medical Home (PCMH) Initiative.

Before joining Network Health, Pano served as Chief Medical Officer of Health Services Partnership in Dorchester, MA, and served as Medical Director for its two member community health centers. Pano also worked for 13 years as physician leader at Health Care Plan in Buffalo, NY, a highly respected staff model HMO. Care delivery transformation was a strategic focus for the staff model organization. The population and individual health focus helped set the groundwork for principles guiding development of Accountable Care Organizations (ACOs) and PCMH. 

Other professional engagements include Senior Clinical and Program Advisor (2016) for the Rhode Island Quality Institute Transforming Clinical Practice Initiative (TCPI) CMMI grant  focused primarily on specialist practice transformation and successful performance under alternative payment models; Senior Clinical Consultant to Northeastern University for a Robert Woods Johnson grant on “Population Health Strategies among Health Plans and Large Provider Organizations; Exploring the Business Case” (2014-2016); and consultant for South County Health on clinical integration (2014). Pano was a member of the Board of Directors at the New England College of Optometry from 2001 to 2022, where he served as Board Chair from 2016 to 2021.
